Patient's Charter of Rights

1.       രോഗികളുടെ അവകാശങ്ങൾ

രോഗിൾക്കും അവരുടെ പ്രതിനിധിക്കും ക്ലിനിക്കൽ സ്ഥാപനവുമായി ബന്ധപ്പെട്ട് ഇനിപ്പറയുന്ന അവകാശങ്ങളുണ്ട്-

PATIENTS’ RIGHTS

patients and their representative have the following rights with respect to the clinical establishment

i. രോഗത്തിൻറെ സ്വഭാവം, രോഗ കാരണം, ,നിർദ്ദേശിക്കപ്പെട്ട പരിശോധനകൾ ചികിത്സ   പ്രതീക്ഷിക്കുന്ന ഫലങ്ങൾ, സാധ്യമായ സങ്കീർണതകൾ, പ്രതീക്ഷിക്കുന്ന ചെലവുകൾ എന്നിവയെക്കുറിച്ചുള്ള കൃത്യമായ വിവരങ്ങൾ അറിയാൻ അവകാശമുണ്ട്;

i. To adequate relevant information about the nature, cause of illness, proposed investigations  and care, expected results of treatment, possible complications and expected costs;

ii. ഓരോ സേവനത്തിനും ഈടാക്കുന്ന നിരക്കുകളെ കുറിച്ചും ലഭ്യമായ സൌകര്യങ്ങളെ കുറിച്ചും അറിയാൻ അവകാശമുണ്ട്..

ii. To information on the Rates charged for each type of service provided and facilities available

iii. ചികിത്സാ രേഖകൾ, പരിശോധനാ റിപ്പോർട്ടുകൾ ഇനം തിരിച്ചുള്ള വിശദമായ ബില്ലുകൾ എന്നിവയുടെ പകർപ്പ് ലഭിക്കാൻ അവകാശമുണ്ട്.

iii. To access a copy of the case papers, patient records, investigation reports and detailed bill (itemized).

iv. ശസ്ത്രക്രിയ, കീമോതെറാപ്പി തുടങ്ങിയ പ്രത്യേക ചികിത്സകൾക്കും പരിശോധനകൾക്കും  മുമ്പായി രോഗിയുടെ അറിവോടെയുള്ള സമ്മതം വാങ്ങിയിരിക്കണം.
iv. To informed consent prior to specific tests/treatment (e.g. surgery, chemotherapy etc.)

v. ചികിത്സിക്കുന്ന ആശുപത്രി നൽകുന്ന രേഖകളും വിവരങ്ങളും ഉപയോഗിച്ച് രോഗികൾക്ക് ഇഷ്ടമുള്ള ഉചിതമായ മറ്റൊരു ഡോക്ടറിൽ നിന്ന് രണ്ടാമത്തെ അഭിപ്രായം തേടുന്നതിന് അവകാശമുണ്ട്.

v. To seek second opinion from an appropriate clinician of patients’ choice, with records and information being provided by the treating hospital.

vi. ചികിത്സാവേളയിൽ രോഗിയുടെ അന്തസ്സും സ്വകാര്യതയും രഹസ്യാത്മകതയും സംരക്ഷിക്ക പ്പെടണം..

vi. To confidentiality, human dignity and privacy during treatment.

vii. ഒരു പുരുഷ ഡോക്ടർ സ്ത്രീ രോഗിയെ ശാരീരികമായി പരിശോധിക്കുമ്പോൾ മറ്റൊരു സ്ത്രീയുടെ സാന്നിധ്യം ഉറപ്പാക്കാൻ അവകാശമുണ്ട്..

vii. To have ensured presence of a female person, during physical examination of a female patient by a male practitioner.

viii. എച്ച്ഐവി ബാധിതനാണെന്ന കാരണത്താൽ ചികിത്സയിലോ പെരുമാറ്റത്തിലോ വിവേചനം കാണിക്കാൻ പാടില്ല..

viii. To non-discrimination about treatment and behaviour on the basis of HIV status

ix. ബദൽ ചികിത്സ ലഭ്യമാണെങ്കിൽ അവ തിരഞ്ഞെടുക്കാൻ.അവകാശമുണ്ട്.

ix. To choose alternative treatment if options are available

x. ഒരു രോഗിയുടെ മൃതദേഹം വിട്ടുനൽകുന്നത് തടയാൻ ഒരു കാരണവശാലും ആശുപത്രികൾക്ക് അധികാരമില്ല.

x. Release of dead body of a patient cannot be denied for any reason by the hospitals.

xi. മറ്റൊരു ആശുപത്രിയിലേക്ക് മാറ്റാനോ/ആശുപത്രിയിൽ നിന്ന് ഡിസ്ചാർജ് ചെയ്യാനോ ആഗ്രഹിക്കുന്ന രോഗി “നിശ്ചയിച്ച ചികിത്സാ തുക നൽകാൻ” ബാധ്യസ്ഥനാണ്..

xi. It is recommended that patient seeking transfer to another hospital/discharge from a hospital will have the responsibility to” settle the agreed upon payment”.

xii.രോഗം, മതം, ജാതി, ലിംഗഭേദം (ട്രാൻസ്ജെണ്ടർ), പ്രായം, ലൈഗിക അഭിരുചി, ഭാഷ, പ്രദേശം എന്നിവയുടെ അടിസ്ഥാനത്തിൽ ചികിത്സയിൽ വിവേചനം പാടില്ല.

xii. It may be specified in the charter that no discrimination in treatment based upon his or his illness or conditions, including HIV status or other health condition, religion ethnicity, gender (including transgender), age, sexual orientation, linguistic or geographical/social origins.

xiii. മെഡിക്കൽ രേഖകൾ ഡിജിറ്റലൈസ് ചെയ്യുന്നതിന് മുമ്പ് രോഗിയുടെ സമ്മതം വാങ്ങണം.

xiii. Informed consent of patient should be taken before digitization of medical records.

xiv. നിശ്ചയിച്ച നിരക്കുകൾ പ്രകാരം ചികിത്സ ലഭിക്കാൻ അവകാശമുണ്ട്.

xiv. Right to care according to prescribed rates wherever relevant

xv. മരുന്നുകളും പരിശോധനകളും തിരഞ്ഞെടുക്കുവാനുള്ള അവകാശം

xv. Right to choose source for obtaining medicines or tests

xvi.മരുന്ന് പരീക്ഷണത്തിൽ പങ്കെടുക്കുന്ന രോഗിക്ക് നിയമപരമായ സംരക്ഷണത്തിനും നഷ്ടപരിഹാരത്തിനും അവകാശമുണ്ട്.

xvi. Right to protection and compensation for patients involved in clinical trials, as per Drugs and Cosmetics Act and other Government Guidelines.

xvii. ബയോമെഡിക്കൽ ഗവേഷണങ്ങളിൽ പങ്കെടുക്കുന്നവർക്ക് ഐ.സി.എം.ആർ മാർഗ്ഗനിർദ്ദേശ പ്രകാരം സംരക്ഷണത്തിനും നഷ്ടപരിഹാരത്തിനും അവകാശമുണ്ട്.

xvii. Right to protection and compensation for participants involved in biomedical and health

research as per ICMR and other Government Guidelines.

xviii. രോഗത്തെകുറിച്ചും ചികിത്സയെകുറിച്ചും ബോധവൽക്കരണത്തന് അവകാശമുണ്ട്.

xviii. Right to Patient Education

xix.പരാതി ക്ൾക്കാനും പരിഹാരം തേടാനുമുള്ള അവകാശം:

xx. ശരിയായ റഫറലിനും ട്രാൻസ്ഫറിനുമുള്ള അവകാശം,

xx. Right to proper referral and transfer, which is free

2.       രോഗികളുടെ ഉത്തരവാദിത്വങ്ങൾ  PATIENTS’ RESPONSIBILITIES

1.ആരോഗ്യവുമായി ബന്ധപ്പെട്ട എല്ലാ വിവരങ്ങളും ഡോക്ടറെ കൃത്യമായി അറിയിക്കുക

Provide all health related information

2.പരിശോധനകളിലും ചികിത്സയിലും ഡോക്ടർമാരുമായി സഹകരിക്കുക.

Co-operate with doctors during examination, treatment.

3.ഡോക്ടറുടെ നിർദ്ദേശങ്ങൾ കൃത്യമായി പാലിക്കുക.

Follow all instructions of Doctors.

4.ആശുപത്രി ഫീസ് കൃത്യമായി നൽകുക

Pay hospital agreed fees on time.

5.ഡോക്ടർമാരുടെയും മറ്റ് ജീവനക്കാരുടെയും അന്തസ്സിനെ മാനിക്കുക.

Respect dignity of Doctors and other hospital staff

6.യാതൊരു കാരണവശാലും ആശുപത്രിയിൽ അക്രമം അഴിച്ച് വിടരുത്

Never resort violence.
